By attending the 2026 Potawatomi Gathering of Nations (the “Gathering”), I agree to this consent, assumption of risk, and release / waiver of claims on behalf of myself and any minor person under age 18 in attendance for whom I am responsible (“Child”). By going to the Gathering, I understand that Forest County Potawatomi Community (“FCP”) is hosting the Gathering and will be taking photographs of the event and participants. As a courtesy / convenience to attendees, FCP provides transportation by bus, van, car, or golf cart to and from the primary location of the Gathering, the FCP Community Center, Bodwewadmi Ktegan (the Forest County Potawatomi Farm), and associated campgrounds.
By attending the Gathering and / or using FCP transportation, I understand and agree that I and / or Child may be in photographs, and that there may be serious risks of harm, injury, or death. I assume responsibility for those risks. I expressly release and agree to hold harmless the Forest County Potawatomi Community and its officers, directors, officials (including elected officials), representatives, agents, employees, vendors, and volunteers (collectively “affiliates”) from any and all liability, loss, claims, causes of action and demands that could be asserted as a result of, or arising during or in connection with my and/or Child’s attendance and / or participation in the Gathering and related events, 25M1461E Legal Approval of 2026 Potawatomi Gathering Waiver Form 4926-9304-2831 v.1.docx and / or use of FCP transportation. I waive any potential claims against FCP, its members, employees, and its affiliates.
I understand and expressly agree that FCP is not responsible for my or Child’s care or welfare, and I consent to participation in activities at the Gathering or any FCP location and / or use of FCP transportation and/or use of photographs of me or Child at such locations. I assume responsibility for all associated risks. Wearing the designated transportation wristband is conclusive proof that I have authorized Child to use FCP transportation. I agree that FCP is not responsible for my or Child’s care, custody, or welfare, and that the above consent, assumption of risk, and release / waiver applies equally to me and to any such Child.
